The Restorative Care Pathway sits within the Support at Home program as a distinct short-term pathway. It provides funding for a tailored program of allied health services over a maximum of 16 weeks. Each episode comes with around $6,000 in funding, and an additional unit of up to $12,000 may be approved for eligible participants with more intensive needs. A team of health professionals works with you to identify what you have lost, set realistic goals, and build a therapy program to help you get there.

What makes it different from ongoing aged care support:

Weeks 14 to 16: Review and Transition
