Hoarding and squalor are not the same problem

Treating them as one is why so many clean-ups fail and the home is back to the same state within a year.

Hoarding disorder is a recognised mental health condition involving persistent difficulty discarding possessions and distress at the thought of doing so. The clutter is a symptom. Removing it without addressing the underlying condition removes the symptom temporarily and frequently damages the relationship that made access possible.

Squalor describes the state of the environment: accumulated waste, contamination, pest infestation, non-functioning utilities. It can occur without hoarding, for example where illness, cognitive decline or disability has made cleaning impossible, and in that case a clean-up plus ongoing domestic assistance genuinely does solve it.

The two often co-occur, and the distinction decides the plan. A squalor clean-up with a support worker afterwards is a maintenance problem. Hoarding needs psychological support alongside any clean-up, usually cognitive behavioural therapy adapted for hoarding, or the clutter returns.
