NDIS Journey, From Access to Better Outcomes

This roadmap breaks the NDIS process into clear steps, so participants and families can take the right action at the right time.

Step 1 Check your eligibility

  • 1 Check age, residency, and disability criteria
  • 2 Write down daily functional impact, what is hard, how often, and what support is already in place
  • 3 Gather medical and allied health evidence with current dates
  • 4 Keep all reports in one folder before applying

Step 2 Submit access request

  • 1 Submit an Access Request through your LAC, NDIA, or support team
  • 2 Attach evidence that describes functional impact, not only diagnosis
  • 3 Track submission dates and reference numbers

Step 3 Plan your goals

  • 1 Prepare for planning or implementation meeting
  • 2 Set practical goals for daily life, work, study, social participation, and health supports
  • 3 Bring examples of risks, breakdown points, and unmet support hours
  • 4 Discuss management type, self-managed, plan-managed, or NDIA-managed

Step 4 Understand your plan

  • 1 Review approved supports line by line
  • 2 Confirm funding categories and flexibility rules
  • 3 Check funding dates, service bookings, and plan duration
  • 4 Flag any obvious funding gaps early

Step 5 Choose your providers

  • 1 Shortlist providers that match goals, location, availability, and communication needs
  • 2 Compare service agreements, cancellation terms, and travel charging
  • 3 Start with one or two key supports before expanding

Step 6 Track your progress

  • 1 Set up monthly tracking for spend and outcomes
  • 2 Collect short progress notes from providers and allied health
  • 3 Keep incident, hospital, and crisis information for evidence

Step 7 Prepare for review

  • 1 Prepare for reassessment early, ideally 10-12 weeks before plan end
  • 2 Summarise goals achieved, unmet supports, and changed needs
  • 3 Submit updated evidence with clear recommendations

Step 8 Request a review

  • 1 If the decision does not meet needs, request an internal review within required timeframes
  • 2 Seek advocacy or specialist support where needed
  • 3 Update provider mix and restart the cycle with better data
