What is a Home Medicines Review?

A Home Medicines Review (HMR) is a free, GP-referred service where an accredited pharmacist visits a patient at home to check all their medications for interactions, dosing problems and adherence issues, then reports findings back to the GP. The service is funded by the Australian Government through the Community Pharmacy Agreement and administered by the Pharmacy Programs Administrator (Department of Health and Aged Care, 2025).

How to choose a provider for this service

Which program pays for a Home Medicines Review, and who qualifies?
