How we rank providers
Rankings in New South Wales are based on real outcomes between providers and families on our platform. They are recalculated daily and cannot be purchased or influenced by advertising.
- How this list is built. Providers shown here are registered with the NDIS for Community nursing care for high needs, or offer nursing and allied health. Generalist providers who only mention this service in passing are excluded, so the list reflects genuine, demonstrated capability.
- Outcomes with families. We measure what happens after a family contacts a provider. Providers where families report positive outcomes rank higher. Multiple signals are weighted across a rolling window.
- Service match. Providers are ranked by how closely their registered services and capabilities match what you are searching for.
- Registration and compliance. NDIS registered and government-approved aged care providers are weighted for meeting quality and safeguards standards.
- Local presence. Providers confirmed in New South Wales rank above those covering only the broader region.
What "Trusted" means. The Trusted badge is awarded to providers with a consistent record of positive outcomes with families on our platform. It is based on multiple behavioural signals and family feedback, and it cannot be purchased.

How to check a provider's credentials
Carevo lists the registration details a provider reports and links you to the official Australian registers so you can confirm them yourself. Here is what each listing shows and where to check it. A listing on Carevo is not an endorsement.
NDIS registration
Listings show whether a provider reports being NDIS registered. You can confirm a provider's current registration and approved support types yourself on the NDIS Commission's public provider register.

What is a Home Medicines Review?
A Home Medicines Review (HMR) is a free, GP-referred service where an accredited pharmacist visits a patient at home to check all their medications for interactions, dosing problems and adherence issues, then reports findings back to the GP. The service is funded by the Australian Government through the Community Pharmacy Agreement and administered by the Pharmacy Programs Administrator (Department of Health and Aged Care, 2025).
How to choose a provider for this service
The list above is ranked using the criteria explained in the methodology section. Four checks matter most when you compare accredited pharmacists yourself.
Registration. Confirm current registration with the Pharmacy Board of Australia via AHPRA, plus specific HMR accreditation, a separate credential from general pharmacy registration.
Response time. HMR referrals move fastest when your GP submits them promptly, so check the response time by state table further down this page before you book.
Pricing transparency. A standard HMR funded through the Community Pharmacy Agreement has no charge to you, so query any request for an upfront fee and confirm the pharmacist bills the Pharmacy Programs Administrator directly.
Coverage. Carevo lists 7,072 allied health providers nationally, so confirm the pharmacist you are matched with holds current HMR accreditation and visits your postcode, a smaller pool than general community pharmacy.
Which program pays for a Home Medicines Review, and who qualifies?
A Home Medicines Review is funded through the Community Pharmacy Agreement and administered by the Pharmacy Programs Administrator, not Medicare, NDIS or Support at Home (Department of Health and Aged Care, 2025). Your GP refers you directly, and there is no cost to you for a standard review. Eligibility is based on clinical need rather than a funding category: you generally qualify if you take five or more regular medications, have recently left hospital, or your GP considers your regimen complex enough to warrant review. DVA card holders access the same service through separate DVA arrangements. NDIS participants and Support at Home clients typically still use this GP referral pathway for the pharmacist review itself, since neither program funds medicine reviews directly.
Availability across Australia
Carevo lists accredited pharmacists across 4,782 suburbs nationally as part of its wider 7,072-strong allied health provider network, though HMR-accredited pharmacists are a smaller subset available by GP referral rather than direct booking. If your GP identifies an ongoing medication-management need alongside the review, compare community nursing care providers, or see the allied health directory for related disciplines.
