Older people taking five or more regular medicines are the core group, along with anyone discharged from hospital in the past month with a changed medication list, and anyone whose new symptom looks like a side effect. Falls, dizziness, confusion, constipation and daytime drowsiness are all common medicine-driven problems in older people, and they are routinely misread as ageing. What a Home Medicines Review actually changes

The government pages explain the programme. What they do not describe is what a review finds, which is the reason to ask for one.

The pharmacist looks at every medicine in the house, not just the current prescription list: repeat scripts, over-the-counter products, supplements and anything left over from a previous prescriber. That physical inspection is the point, and it is why the review happens at home.

The common findings are consistent. Duplication, where two prescribers have started the same drug class. Interactions between a prescription medicine and a supplement or over-the-counter product nobody recorded. Doses that no longer suit, particularly renally cleared drugs as kidney function declines with age. Anticholinergic burden, the accumulated sedating and cognitive load of several ordinary medicines taken together. Practical barriers: bottles that cannot be opened, labels that cannot be read, an insulin pen the person cannot operate.

The pharmacist reports to your GP with recommendations, and the GP decides. Nothing changes without the prescriber, which is why the review is safe to request.
