How much does post-hospital nursing cost in Tasmania?

Post-Hospital Nursing in Tasmania has this published pricing context: Transition Care Programme is government subsidised; private nursing and personal care rates set by each provider. Actual rates vary by provider, funding type, and the level of support required.

Funded rates work differently: NDIS supports are capped nationally (see the cost breakdown below), while Support at Home and CHSP prices are set by each provider. The range above is what providers commonly charge.

Workers delivering NDIS supports in Tasmania need the NDIS worker screening through the Registration to Work with Vulnerable People scheme, administered by the Tasmanian Government. 82% of post-hospital nursing providers listed in TAS are registered with the NDIS Quality and Safeguards Commission. Unregistered providers can still deliver many supports if your plan is plan managed or self managed.

NDIS price caps do not change by state: the NDIS Pricing Arrangements and Price Limits set the same maximum rates in Tasmania as everywhere else in Australia.

What is post-hospital care?

Post-hospital care is short-term support at home in the weeks after a hospital stay: the period when wounds need dressing, medications have changed, strength has not come back, and the risk of ending up back in hospital is highest. A typical arrangement combines home nursing for wound care and medication management, personal care for showering and dressing, domestic assistance while lifting and bending are off-limits, and allied health such as physiotherapy to rebuild strength and prevent falls. It is usually intensive for two to twelve weeks, then tapers off or transitions into ongoing support.

What government programs cover care after hospital?

Two pathways matter for older people. The Transition Care Programme provides up to 12 weeks of subsidised, goal-oriented care directly after a hospital stay, and you must be assessed while still in hospital, so raise it with the ward’s discharge planner before you leave (Department of Health and Aged Care, 2025). If you are already in the aged care system, the restorative care pathway under Support at Home funds allied-health-led recovery episodes. For NDIS participants, supports connected to the hospital stay are coordinated through the plan, and hospital liaison officers help arrange discharge supports. Veterans with a DVA card should start with DVA-funded home care.

What if I can’t wait, or don’t qualify?

Carevo measures whether a provider accepts an enquiry, not when care starts or a practitioner contacts you. Confirm current capacity and a start date directly. How Carevo measures enquiry acceptance.

Who actually arranges care after a hospital discharge?

In theory, the hospital’s discharge planner. In practice, responsibility is split and things fall through the cracks in the handover. The discharge planner or ward social worker arranges the clinical follow-up and can refer into the Transition Care Programme, but only if the assessment happens before discharge. The GP takes over medical oversight after discharge but does not arrange home support. My Aged Care and the NDIS run their own assessment timelines that rarely match the discharge date. What usually closes the gap is a family member who books interim support directly, which is exactly what the ranked list above is for. If you are reading this from a hospital corridor, the two questions to ask the ward today are: has an aged care or TCP assessment been requested, and what date is discharge actually planned for.

How long does post-hospital care last?

Plan in three phases. The first two weeks carry the highest readmission risk and usually need daily support: nursing for wounds and medications, personal care, and meals. Weeks three to six taper as strength returns, often down to a few visits a week with physiotherapy doing the heavy lifting. By week twelve, government short-term programs end (the Transition Care Programme runs up to 12 weeks), and the honest question is whether support is still needed. If it is, the need is ongoing rather than post-hospital: that is the point to get a My Aged Care assessment for a Support at Home budget rather than keep buying week-to-week, because assessment and budget allocation take time and starting the process at week twelve leaves another gap.

How to choose a post-hospital provider from the list above

The list above is ranked using the criteria explained in the methodology above it. For post-hospital support specifically, check four things. Clinical capability first: confirm the provider can supply a nurse for wound care and medication management, not just support workers, and that nurses hold current AHPRA registration. Speed second: ask whether they can genuinely start this week, because a start date in three weeks defeats the purpose. Communication third: the provider should take the discharge summary and follow it, and flag deterioration to the GP. Escalation last: ask what the provider does if a wound looks infected or pain is not controlled on a Sunday night.

What does post-hospital care reduce the risk of?

The weeks after discharge are when falls, medication errors and missed follow-up appointments put people back in hospital. Structured support targets each one: supervised movement and home hazard checks against falls, a nurse reconciling the new medication list against what is actually in the cupboard, and transport to follow-up appointments so specialist reviews are not skipped. If recovery stalls and support needs look permanent rather than short-term, that is the point to get a My Aged Care assessment for an ongoing budget, or to compare overnight care for nights.

Availability

Carevo lists 32,033 NDIS providers and 4,325 aged care providers across 8,918 suburbs nationally, with records updated 2026-08-26. Nursing is named in 324 of the 9,483 tracked inquiries on Carevo, and post-hospital arrangements are among the most time-critical, so response speed weighs heavily in the ranking above.

How this page is compiled

The provider rankings and figures on this page come from Carevo platform records, including provider registrations, measured response times and coverage, together with the cited official sources. Carevo is an Australian platform that connects families with NDIS and aged care providers. Carevo does not deliver care itself.

Funding sources and checks

Check the current NDIA pricing arrangements and Support at Home program rules for the proposed service. Funding depends on the approved support and available budget; a Carevo listing is not approval. Confirm availability and a start date directly with the provider.

How we rank providers

Rankings in Tasmania use directory relevance and observable activity on Carevo: service match, distance, register-sourced status fields, whether an enquiry can reach the provider, and recent response and connection activity. Rankings are recalculated daily. They do not assess care quality or recommend a provider.

  • How this list is built. Providers shown here offer nursing and personal care.
  • Activity on Carevo. We measure recent actions such as connecting on an enquiry, opening its contact details, responding, and an inquirer reporting whether a connection helped. These are platform activity signals, not an assessment of the support delivered.
  • Service match. Providers are ranked by how closely their registered services and capabilities match what you are searching for.
  • Register-sourced status. NDIS registration fields come from the NDIS Commission provider register. Aged care approval fields come from the My Aged Care register. These statuses are ranking inputs, not Carevo endorsements, and you should confirm the current register entry before choosing a provider.
  • Carevo enquiries. Providers who have paused requests from us are moved down among providers with the same relevance. They stay visible and are marked as not receiving Carevo enquiries.
  • Recent activity. Providers active on Carevo recently (a connection, a login, or a profile update) get a small boost, the same signal shown as "Active this week" on their card. It is deliberately small: stronger relevance and condition evidence outweigh a recent login.
  • Local presence. Providers confirmed in Tasmania rank above those covering only the broader region.

What the enquiries status means

It answers one question: would a request sent through Carevo today actually reach them? It is not a rating, it is not about the provider's books, and it is not a statement about their capacity, their waiting list, or the quality of their care.

Receiving
At least one contact on their account is set up to receive new requests from us, and someone has signed in recently enough for us to say a person is there.
Not receiving
Every contact on the account has unsubscribed or paused, so we have no way to pass an enquiry on.
No status shown
We cannot tell either way, so we say nothing rather than label the provider. The account is set to receive enquiries, but nobody has signed in recently, or there are no contact details on file. It is not a negative mark, and most listings sit here.

A provider who is not receiving our enquiries may well be taking new clients directly. Providers set this themselves and can turn it back on at any time from their Carevo account or from any email we send them. Providers not receiving our enquiries are moved down the list and never hidden. It is refreshed daily, so a provider who resumes, or signs in, reappears within a day.

What the Carevo activity rating means

The Strong activity rating describes how a provider behaves on Carevo, not the care they deliver.

Where it starts
Every provider starts at a baseline of 50.
What moves it
Enquiries from the last 90 days. The strongest positive is a care seeker naming that provider as the one who helped them. The clearest negative is connecting on an enquiry and never opening the contact details.
How it is kept honest
Recalculated daily from scratch, shown only above the baseline and only with at least two scored signals, and removed automatically when either stops being true. It cannot be purchased.

It is not an endorsement, a recommendation, or an assessment of care quality, and we do not screen providers for complaints. Complaints are handled by the NDIS Commission and the Aged Care Quality and Safety Commission. See the full activity rating methodology.

How many post-hospital nursing providers are there in Tasmania?

There are 346 post-hospital nursing providers with active listings in Tasmania (TAS) on Carevo. Providers are ordered using service match, location, register-sourced status and observable activity on Carevo. No provider can pay for a higher position, a badge or an activity rating.
