A reporting structure for an appropriately qualified practitioner to complete using their own assessment and records. It contains no test instrument, diagnosis, clinical scoring or predetermined recommendation.

What is in the download

Context evidence and progress

Practitioner and report. Person and purpose. Goals and starting point. Assessment evidence. Intervention delivered. Progress. Barriers and risks.

Recommendations and communication

Clinical reasoning. Proposed support. Alternatives. Consequences and uncertainty. Funding context. Person involvement. Practitioner sign-off. Follow-up.

How to use it

  1. Save a working copy and replace the bracketed prompts. Remove sections that do not apply.
  2. Involve the person in the discussion and use their preferred communication format.
  3. Verify dates, contacts and source records. Clearly attribute information supplied by someone else.
  4. Record who completed the document and when. Make later corrections traceable.
  5. Share only necessary information with the appropriate recipient and retain the record securely.

Before you rely on it

Use the specific NDIA, Medicare, DVA, insurer or receiving-service requirements that apply. This is a report scaffold, not a universal referral, functional capacity assessment or discharge protocol. Assessment, diagnosis and recommendations remain the qualified practitioner’s responsibility. Do not send unnecessary raw clinical notes.
