How to use it

- Record who completed the document and when. Make later corrections traceable.

Check report due dates in the request for service and provider portal. The NDIA publishes reporting templates; use the appropriate official template when preparing a requested report. This log helps gather evidence but does not submit anything. Record actual time and check claim rules separately.
