Where can I find complex care providers in Australia?

There is no single official directory of complex care providers. The NDIS Quality and Safeguards Commission’s register shows which organisations hold registration for High Intensity Daily Personal Activities and Community Nursing Care, and My Aged Care’s Find a provider tool covers registered aged care providers, but neither shows who actually rosters clinical staff in your suburb. The ranked list above narrows the field to providers with current coverage and measured response times on the Carevo platform.

What is complex care?

Complex care is home-based clinical support for people with high-level or multiple medical needs, such as ventilator management, tracheostomy care, enteral tube feeding, complex wound management or multi-drug medication regimens. It is delivered by registered nurses and specially trained support workers rather than general personal care staff, and on the NDIS it is funded through specific nursing line items including Community Nursing Care and High Intensity Daily Personal Activities (NDIS Pricing Schedule 2026-27).

What makes care “complex” is clinical risk rather than hours. Supports such as PEG feeding, seizure management for high-risk epilepsy, subcutaneous injections and complex bowel care can cause serious harm if performed by an untrained worker. Severe behaviours of concern sit in the same category: supporting them safely means workers trained in the person’s positive behaviour support plan, not simply extra staffing.

What makes an NDIS support “high intensity”?

The NDIS treats a daily personal activity as high intensity when delivering it safely requires additional training and skill. The NDIS Commission publishes skills descriptors for these supports, covering complex bowel care, enteral feeding, severe dysphagia, tracheostomy care, urinary catheter management, ventilation, subcutaneous injections and complex wound care (NDIS Quality and Safeguards Commission, 2025). Providers claiming high-intensity rates must show their workers meet the relevant descriptor for each task.

The price difference is real. A standard weekday daytime support hour is capped at $73.58, while the high-intensity weekday daytime cap is $79.60. If a provider bills the higher rate, ask what training the rostered workers hold; the rate pays for that competence.

How to choose a provider from the list above

The list above is ranked using the criteria explained in the methodology above it. Four checks matter most when narrowing down a complex care provider.

Registration. Confirm NDIS Quality and Safeguards Commission registration specifically covers High Intensity Daily Personal Activities or Community Nursing Care, since general NDIS registration does not guarantee this approval.

Pricing transparency. Request an itemised quote benchmarked against NDIS price limits rather than a flat estimate, with standard, high-intensity and nursing hours listed separately.

Coverage. Verify the provider has rostered clinical staff actually servicing your area, not just a listed service radius, and ask how many workers are trained on your supports so one resignation does not collapse the roster.

How much does complex care at home cost?

Most NDIS complex care hours are billed as high-intensity support work capped at $79.60 an hour for weekday daytime, with evening, weekend and public holiday loadings above that. Active overnight support is capped at $82.57 an hour on weekday nights, while a sleepover shift, where the worker sleeps at the home and wakes for occasional assistance, is billed at $311.79 a night. Nurse-delivered tasks are billed under separate nursing line items at higher rates.

For older people, Support at Home funds clinical care at no cost to the participant within the classification budget (Department of Health and Aged Care, 2025). Be wary of flat weekly quotes that do not show which hours are standard, high intensity or nursing; that is where overcharging hides.

Which funding pays for complex care, and who qualifies?

NDIS participants access complex care through specific nursing supports, primarily Community Nursing Care and High Intensity Daily Personal Activities, not generic Core Supports funding. Eligibility depends on the clinical need being disability-related; the NDIS does not fund supports that are the health system’s responsibility, such as acute treatment or general palliative care (NDIS Pricing Schedule 2026-27). Older Australians with clinical needs, such as wound care or medication management, can instead access Support at Home, where clinical care is funded 100% by government within the participant’s classification budget, up to Classification 8 at roughly $80,294 a year (Department of Health and Aged Care 2025). A My Aged Care assessment sets Support at Home eligibility and classification level. Veterans with a Gold or White Card can compare DVA-funded home care instead.

Who supervises complex care workers?

Safe complex care runs on delegation. A registered nurse assesses the task, trains the support worker on the participant’s routine, signs off on competence and reviews it on a schedule, staying clinically accountable for what they delegate. Nurses must hold current AHPRA registration (AHPRA, 2026). When you compare providers, ask three things: who is the supervising RN, how often they reassess worker competence, and what happens at 2am when something falls outside the worker’s delegation. Providers that deliver home nursing alongside support work can usually answer directly because the RN is in-house rather than subcontracted.

Can I get 24 hour complex care at home?

Yes, and the roster model matters as much as the provider. Around-the-clock active care usually means two or three workers rotating through the day plus an active overnight shift, which suits ventilation, frequent repositioning or unpredictable seizures. Where nights are mostly standby, a sleepover arrangement through overnight care costs far less than active night hours. Live-in care places one consistent worker in the home for blocks of days and works well when clinical tasks are scheduled rather than continuous. Most 24 hour arrangements combine these models, and a good provider will map your night-time clinical needs before quoting.

Crisis and complex needs support

A crisis arrangement is needed when an existing roster, placement or family support breaks down and there is an immediate safety risk. Complex care providers may be able to assemble trained staff, nursing oversight and an after-hours escalation plan, but Carevo is not an emergency service. Call 000 where there is immediate danger or a medical emergency.

For an urgent but non-emergency transition, tell each provider the required start date, clinical tasks, behaviour support plan, medication needs, staffing ratio and overnight model. Ask who owns the written risk plan and how the temporary roster will become stable. Who can legally deliver complex care at home

This is the question that matters most, because the answer decides whether an arrangement is safe or merely cheap.

High intensity daily personal activities include complex bowel care, enteral feeding, tracheostomy management, ventilation support, urinary catheter management, subcutaneous injections, complex wound care and severe dysphagia management (NDIS Quality and Safeguards Commission, 2025).

A support worker can perform many of these, but only through delegated care: a registered nurse assesses the task, trains the worker on that specific person’s routine, signs off their competence in writing, and sets a review schedule. Without that chain, a support worker performing a clinical task is operating outside their scope and the provider is carrying a risk they may not have insured.

That delegation is also what justifies the high intensity rate of $79.60 an hour on a weekday daytime, against $73.58 for standard support (NDIS Pricing Schedule 2026-27).

Ask any provider quoting complex care: which registered nurse holds clinical oversight, when was this worker signed off for this task for this person, how often is competency reviewed, and who is called at 2am. A provider that cannot answer all four is charging a clinical rate without the clinical structure.

Building a layered roster that does not run out of budget

Complex care budgets fail in a predictable way: everything is rostered at the highest skill level, and the funding runs out two thirds through the plan.

A well-built roster layers three tiers. Registered nurse time for assessment, care planning, delegation and review, plus any task that cannot be delegated. Enrolled nurse or trained support worker time for the delegated clinical tasks on a stable routine. Standard support worker time for everything that is not clinical, which is usually most of the day.

The saving is not in cutting care. It is in not paying an RN rate for personal care, meals and community access because the same worker happens to be present.

Two practical points. Ask for the roster in half-hour blocks with the skill level named against each, the same way a SIL roster of care is built. And review the delegation quarterly, because a task that was unstable at plan start is often stable six months later and can move down a tier safely.
