What is occupational therapy at home?

Home-based occupational therapy is therapy delivered by a university-trained, AHPRA-registered practitioner in a client’s own home, covering safety assessments, daily living skills training, and equipment or home modification recommendations. It suits people recovering from injury or stroke, managing disability, or ageing in place who need practical strategies for showering, cooking, or moving safely at home (NDIS Quality and Safeguards Commission, 2025).

How to choose a provider for this service

The list above is ranked using the criteria explained in the methodology above it. Four checks matter specifically when you compare occupational therapy providers.

Registration. Confirm the OT holds current AHPRA registration and check NDIS Commission status if you’re agency-managed; self-managed and plan-managed participants can use any qualified OT.

Response time. Assessments often follow a hospital discharge or a plan review deadline. Benchmark actual reply speed against the response-time table further down this page, not a stated turnaround.

Pricing transparency. Ask for a rate card separating the hourly rate from travel, report and equipment fees, and stating whether it bills to Capacity Building or Support at Home.

Coverage. Occupational therapy is named in 44 of the 9,483 tracked inquiries on Carevo, but OTs aren’t rostered in every suburb; confirm the provider has clinical staff actually covering your area before booking.

What does an OT home assessment cover?

An in-home assessment watches how you actually manage tasks rather than how you describe them in a clinic. The therapist observes transfers on and off the bed, toilet and shower access, kitchen tasks, steps, entries and lighting, screens falls risk, and measures where rails or equipment would go. The output is a written report with recommendations: technique changes, equipment to trial, home modifications, or referral to another discipline. Have your plan goals, discharge summaries and a list of the tasks that have become hard ready before the visit; the report lands closer to what you need funded.

Why do home modifications, equipment and functional reports all start with an OT?

Occupational therapy is the gateway discipline in both funding systems. NDIS home modifications need an OT’s assessment before approval: minor works proceed on the OT’s recommendation, complex modifications need a full assessment plus a building scope (NDIS Quality and Safeguards Commission, 2025). Assistive technology beyond low-cost items needs a written assessment, usually from an OT; see assistive technology assessments. Functional capacity assessments, the reports that anchor plan reassessments, SIL and housing applications, are OT-led. In aged care, the Support at Home AT-HM Scheme funds equipment and home modifications outside the quarterly budget, and most items on its list need a prescription, usually from an OT (Department of Health and Aged Care, 2025). If your year involves equipment, modifications or a major report, book the OT first; every other approval waits on that assessment.

What does an NDIS registered occupational therapist mean?

Two different registrations get run together in that search. AHPRA registration belongs to the individual therapist: every practising occupational therapist in Australia must hold it, and the public register shows their registration status, endorsements and any conditions on practice (AHPRA, 2026). NDIS registration belongs to the provider organisation, which registers with the NDIS Quality and Safeguards Commission under the Therapeutic Supports group, is audited against the NDIS Practice Standards, and appears on the NDIS Provider Finder. A sole-trader OT can register the same way. NDIS registration is only required when the participant’s plan is NDIA-managed; plan-managed and self-managed participants can use any AHPRA-registered OT, and many experienced clinicians work unregistered because the audit is expensive for a small practice. How much does an NDIS occupational therapist cost?

The 2026-27 NDIS Pricing Schedule caps occupational therapy at $193.99 an hour, the same cap as speech pathology and audiology; the per-discipline comparison is on allied health disciplines. The cap covers assessment, session time, report writing and clinical planning, so a home modification report draws on the same Improved Daily Living budget as therapy sessions. Travel for home visits adds 50 percent of the hourly rate. These are caps, not fixed prices; self-managed and plan-managed participants can negotiate below them. Under Support at Home, allied health is typically priced around $185 an hour, with clinical supports government funded at no participant contribution (Department of Health and Aged Care, 2025).

Which funding pays for occupational therapy at home, and who qualifies?

For NDIS participants, occupational therapy is funded under the Capacity Building - Improved Daily Living budget, approved when a therapist’s assessment links the therapy to disability-related need; self-managed and plan-managed participants can engage any qualified OT, while agency-managed participants need a provider registered in the Therapeutic Supports group (NDIS Quality and Safeguards Commission, 2025). For people 65 and over, or 50 and over for Aboriginal and Torres Strait Islander people, Support at Home funds OT as a clinical and independence support once a Single Assessment System assessment sets your ongoing classification level (Department of Health and Aged Care, 2025). Lower-intensity OT linked to a CHSP plan suits people not yet assessed for Support at Home.

What is the difference between paediatric, adult and aged care OT?

The registration is one profession, but caseloads split sharply. Paediatric OTs work on play, school readiness, handwriting, feeding and daily routines, usually coaching parents within the session; children under 9 typically enter through early childhood early intervention. Adult disability OTs carry most of the NDIS assessment load: functional capacity, housing, assistive technology and modification reports. Aged care OTs concentrate on falls prevention, safe transfers, pressure care and equipment for ageing in place. Registration does not distinguish between these caseloads (AHPRA, 2026), so ask which one the assigned therapist carries; a report writer who knows your funding body’s templates produces reports that pass review first time.

Sensory processing and sensory support

Many occupational therapists assess and support sensory processing, which is how a person takes in and responds to sound, touch, movement and other input. Difficulty here affects concentration, sleep, mealtimes and behaviour, and is common for autistic people and children with developmental differences. An OT can build a sensory profile, set up a sensory diet of calming and alerting strategies for home and school, and recommend sensory equipment. Under the NDIS, everyday sensory items are usually funded from Core Supports consumables and higher-cost items through Capital Supports assistive technology, with the OT’s assessment used to justify the request (NDIS Pricing Schedule 2026-27).

What an OT home assessment actually covers

The home assessment is the gateway to most equipment and modification funding, and knowing its shape helps you get a useful report rather than a generic one.

The therapist watches you do the tasks that are causing difficulty, in the place they happen. Not a conversation about them, the actual task. Getting on and off the toilet, in and out of the shower, up the front steps, out of a chair, into bed, making a cup of tea.

From that they document: the specific transfers that are unsafe and why, the environmental features contributing (step height, doorway width, tap type, floor surface), what equipment would resolve each, what modification would resolve what equipment cannot, and the person’s capacity to use each option safely.

The report then goes to whichever funder applies. That is why one visit can produce a shower stool recommendation, a grab rail prescription and a bathroom modification quote request at the same time. Ask the therapist to assess everything in one visit rather than booking separately for each problem, because a second assessment is a second fee.

In-home OT for older Australians: what it costs and who pays

For people 65 and over, occupational therapy is a clinical support under Support at Home, which carries no participant contribution once approved in a care plan after assessment through the Single Assessment System (Department of Health and Aged Care, 2025). This is the single most under-used entitlement in aged care: the assessment that unlocks equipment and modification funding is itself free.

Where someone is not yet on Support at Home, the Commonwealth Home Support Programme funds entry-level allied health, and a GP Chronic Condition Management plan covers up to five rebated allied health visits a year shared across all disciplines.

For NDIS participants the OT hourly cap is $193.99 in 2026-27 (NDIS Pricing Schedule 2026-27), billed from Capacity Building, with travel for home visits charged separately at 50 per cent of the service rate.

The practical sequence for an older person: request an OT assessment through My Aged Care, get the report, then use it to apply for equipment or modifications through the AT-HM Scheme. Doing it the other way round, buying equipment first and seeking funding after, is the most common way people end up paying for the wrong thing.
