The higher price reflects the additional skills and qualifications required. Standard personal care codes should not be used for participants approved at this level.

• A participant purchases an item under this code at the listed price of the quoted rate. The provider submits a claim with the invoice and any supporting documentation.
• A provider supplies equipment and claims the cost directly. If the item exceeds the price limit, the provider must obtain approval for the additional amount before delivery.
