What is the Transition Care Programme?

The Transition Care Programme (TCP) is a jointly funded Commonwealth and state/territory programme that provides short-term, goal-directed support after a hospital stay, so an older person can regain strength and confidence before returning home or moving into longer-term care. Under the Department of Health and Aged Care (2025) TCP guidelines, it runs for up to 12 weeks and includes nursing, allied health, and personal care, delivered at home or in a residential setting.

For an older person in Queensland recovering from a fall, stroke, or major surgery, this extra time with nursing and therapy support often decides whether they can return to their own home rather than move into residential care sooner than they need to.
