What is care management in aged care?
Care management is the ongoing coordination of a person’s Support at Home services: developing and reviewing the care plan, scheduling providers, managing the funded budget, and organising reassessments. Under Support at Home, it is delivered as a clinical support and funded from a pooled amount equal to 10% of each participant’s quarterly budget, with care plans reviewed at least every 12 months (Department of Health and Aged Care, 2025).
For an older person in Tasmania, a care manager is the single point of contact who ties the whole plan together. Instead of the family chasing separate cleaners, nurses and allied health visits, the care manager arranges and reviews them, and adjusts the plan when needs change after a fall, a hospital stay or a new diagnosis. This is often the difference between a package that works and one that sits underused.
Who it is for and how it is funded
Care management is for anyone approved for Support at Home, the program that replaced Home Care Packages from 1 November 2025 (Department of Health and Aged Care, 2025). It is fully government funded with no co-contribution required. Eligibility starts with an assessment through My Aged Care on 1800 200 422; once a person is approved and matched with a provider, care management runs from day one alongside their other funded supports.
Finding a care management provider in Tasmania
When you compare care management providers in Tasmania, ask how often the care manager makes contact, whether you keep the same person over time, and how quickly they respond when something changes. Continuity matters, because a care manager who knows the person’s history and household can act faster than one meeting them cold. It also helps to ask which local services the provider already works with, since established relationships with nurses and allied health in Tasmania can shorten the wait to get supports in place.
