Care management is the ongoing coordination of a person’s Support at Home services: developing and reviewing the care plan, scheduling providers, managing the funded budget, and organising reassessments. Under Support at Home, it is delivered as a clinical support and funded from a pooled amount equal to 10% of each participant’s quarterly budget, with care plans reviewed at least every 12 months (Department of Health and Aged Care, 2025).
