The End-of-Life Care Pathway is a short-term stream within the Support at Home program for people expected to live three months or less. It provides around $25,000 of funding over 12 weeks, extendable to 16 weeks if funds remain, for clinical, personal and household supports that let a person stay at home instead of in hospital (Department of Health and Aged Care, 2025).
