The End-of-Life Care Pathway is a short-term stream within the Support at Home program for people expected to live three months or less. It provides around $25,000 of funding over 12 weeks, extendable to 16 weeks if funds remain, for clinical, personal and household supports that let a person stay at home instead of in hospital (Department of Health and Aged Care, 2025).

The pathway brings the nursing, personal care and household help needed to make that possible into one funded package, so the family is not left arranging each piece separately at a hard time.
