What is the End-of-Life Care Pathway?
The End-of-Life Care Pathway is a short-term stream within the Support at Home program for people expected to live three months or less. It provides around $25,000 of funding over 12 weeks, extendable to 16 weeks if funds remain, for clinical, personal and household supports that let a person stay at home instead of in hospital (Department of Health and Aged Care, 2025).
The aim is practical. Many people would rather spend their last months at home, close to family, than in a hospital ward. The pathway brings the nursing, personal care and household help needed to make that possible into one funded package, so the family is not left arranging each piece separately at a hard time.
Who it is for and how it is funded
The pathway is for people who meet the general aged care entry criteria (65 or over, or 50 or over for Aboriginal and Torres Strait Islander people, or people who are homeless or at risk of homelessness), have a doctor or nurse practitioner estimate of three months or less to live, and an AKPS score of 40 or less. Clinical supports such as nursing and physiotherapy are fully government funded; personal care and everyday living services may attract a participant contribution based on a means-tested assessment (Department of Health and Aged Care, 2025). Access starts with My Aged Care on 1800 200 422, who flag the request as urgent.
For a family in Northern Territory, the funding and eligibility rules are set nationally and do not change with location. What matters locally is whether providers covering Northern Territory have nurses available for after-hours and overnight visits, since end-of-life care rarely keeps business hours.
Finding End-of-Life Care at Home in Northern Territory
Because the pathway runs for a short, fixed period, the speed of getting started matters more than usual. When you speak with a provider in Northern Territory, ask how quickly they can begin, whether they offer 7-day and overnight cover, and how they coordinate with the person’s GP and palliative care team. Continuity is worth asking about too, since a small, consistent group of carers is easier for the person and the family than a rotating roster.
Availability on Carevo
Carevo lists 2,619 aged care providers, matched by the criteria explained above. For the wider range of Support at Home services beyond this pathway, see Support at Home providers.
How this page is compiled
The provider rankings and figures on this page come from Carevo platform records, including provider registrations, measured response times and coverage, together with the cited official sources.
